Important: State of Illinois Health Requirements for the 2024-25 School Year

Please review the follow as certain grade levels have yearly medical form requirements.

These forms are due by October 15, 2024 to avoid exclusion from school and can be directly uploaded to our confidential site.  

All Students (required yearly):

• Saint Joseph School Health History Form: This is required for all students, every new school. 

• Physical Exam (Parents please complete the yes/no questions, sign and date) the exam must be completed within one year prior to the first day of school.
• Complete Immunization Record signed and stamped by a doctor

• Physical Exam  (Parents please complete the yes/no questions, sign and date) the exam must be completed within one year prior to the first day of school.
• Completed Immunization Record signed and stamped by a doctor 
• Dental Form on Illinois Form dated between November 2023-May 15, 2025
• Eye Exam on Illinois Form dated within one year of the first day of school.

• Dental Form on Illinois Form dated between November 2022-May 15, 2024

Sixth Grade:

• Physical Form (Parents please complete the yes/no questions, sign and date) the exam must be completed within one year prior to the first day of school.
• Completed Immunizations Record signed and stamped by a doctor
• Dental Form on Illinois Form dated between November 2022-May 15, 2024

• State of Illinois Physical Form
• Eye Exam Form
• Dental Exam Form (only if transferring into Kindergarten, 2nd grade or 6th grade

• Transfer of current Illinois school health records (current physical exam with proof of updated immunizations, dental and eye exam).

• A yearly sports physical is required for all athletes in 5th-8th in order to participate in school sponsored athletic programs.
• 6th Grade full physical can be used as a sports physical.

All health forms are due on or before October 15, 2024 to the school nurse to avoid exclusion from school.
