Health Advisory: Stop Vaping Now!!
Vaping is a serious problem and it's never to early to share the dangers and facts with your children. The CDC says e-cigarettes should never be used by youth, young adults, pregnant women or adults who do not currently use tobacco products. The American Lung Association warned that "e-cigarettes are not safe" and can cause irreversible lung damage and disease. I have included a link for parents to get the facts and an age appropriate link for teens to learn the dangers of Vaping. Thank you for your time.

BACK TO SCHOOL HEALTH REQUIREMENTS
Welcome back and I hope you had a relaxing and fun summer break! As we get back in the swing of things please take a minute or so to make sure your have your student(s) health requirements in order for the new school year. All health forms are due before October 14, 2019 to avoid exclusion from school.
Preschool (New to SJCS for PreK or first year of PreK)
- State Of Illinois Certificate of Child Health Examination Form filled out by a physician with proof of all required immunizations.
Kindergarten:
- State of Illinois Certificate of Child Health Examination Form filled out by a physician with proof of all required immunizations.
- Eye exam
- Dental exam
Second Grade:
- Dental exam
Sixth Grade:
- State Of Illinois Certificate of Child Health Examination Form filled out by a physician with proof of all required immunizations.
- Dental Form.
Students Grade 5-8th:
- If participating in a school sport, a yearly Sports Physical is required. Sixth grade full physical counts as a sports physical.
New student(s) transferring from out of state:
- State Of Illinois Certificate of Child Health Examination Form filled out by a physician with proof of all required immunizations.
- Eye exam
- Dental exam
New student(s) transferring in from an Illinois school:
- State Of Illinois Certificate of Child Health Examination Form filled out by a physician with proof of all required immunizations.
