New students transferring in to an Illnois school for the first time are required to provide the following forms:
- State of Illinois Certificate of Child Health Examination with proof of up to date immunizations.
- Dental Exam Form (only if entering into K, 2nd or 6th grade)
- Eye Exam Form
- Medication Authorization Form- if applicable and required if medication will be needed during the school day. This includes over the counter or prescription. No medication can be given without a completed Authorization.
