Use this form to upload your child's sports physical form for the 2023-24 school year.

• Sports physicals are good for 395 days from the date issued.
• A sports physical form must be on file with the school nurse before the first practice.
• A school physical exam form may be used in lieu of a sports physical, HOWEVER, a sports physical cannot be accepted as proof of a physical exam for a mandated grade level physical exam (6th graders).
• Please be sure the athlete and parent/guardian sign the form before submitting it to the website.
