Proof of the COVID-19 vaccination, although not required, will be extremely helpful for ensuring vaccinated students, if exposed or if traveling to an orange state on the Chicago Advisory List, are not subject to quarantine requirements. We are encouraging parents to submit their child's completed vaccination card to assist in expediting quarantine decisions and preventing vaccinated students from quarantining unnecessarily. 

If you would like to proactively upload your child's or your COVID-19 vaccination card, you may do so using this secure portal. Only school administration personnel will have access to the vaccination information.

Individuals are considered fully vaccinated 2 weeks after their second dose in a two dose series, such as the Pfizer-BioNTech or Moderna vaccines, or 2 weeks after a single-dose vaccine, such as Johnson & Johnson's Janssen vaccine.
