Please use this page to upload your child's required kindergarten medical forms for the 2023-24 school year. The state of Illinois requires all incoming kindergarten students to have a current Illinois physical exam and up to date immunizations, vision exam, and dental exam for the entrance to kindergarten.

Before uploading your forms, please verify the following:

Physical Exam must be on the State of Illinois Certification of Child Health Examination and done within one year prior to the first day of school. Parents/Guardians must complete, sign and date the Health History Section on the back of the physical exam before submitting.
Up to date Immunization Record- Most children entering Kindergarten will need a DPT/DTap, Polio, MMR, and Varicella immunization.
Vision Exam must be on a State of Illinois Vision Exam Form and done within one year prior to the first day of school.
Dental Exam on an Illinois Dental Form dated between November 15, 2022- May 15, 2024
