Spring Break 2021: Proof of Negative COVID Test

If your child travels to an orange state during Spring Break, they may return to school without a quarantine IF a parent uploads a copy of their child's negative COVID test result. The child's name and date of test must appear on the results.

Proof of a negative COVID test MUST BE SUBMITTED by Sunday, March 28 by 2PM using this form only. We are unable to accept documentation on Monday morning during arrival. We have over 150 students traveling, and it will be far too difficult to look at test results as we are helping students out of the car. Thank you for your compliance with this.
